What Should I Bring With Me to My First Appointment?

It is recommended to bring a list of your current medications as well as a referral from your doctor. As well, we welcome a list of any concerns or questions you may have. Please ensure that you arrive at the office at least 15 minutes prior to your appointment in order to be able to provide us with the necessary insurance information and paperwork required to put you into our computer.
